ASHLYN BOLTON LLC Liability Waiver and Release Agreement

Assumption of Risk

I, the undersigned participant (or the parent/guardian if the participant is under 18), acknowledge that I am voluntarily participating in dance classes and events provided by ASHLYN BOLTON LLC, held at the Barrymore Hotel, located at 111 W Fortune St. Tampa, FL 33602 I understand and accept that participating in high-impact dance classes, including but not limited to Tampa Bay Tap Festival Classes, Performance Track, Mini Bootcamp, Professional Development Program, COPASETIC, Student Showcase, Tap Jam and Wrap Party, Tampa Bay Taps, Rhythm and Sole Tap Ensemble, Youth Tap Crew, and other workshops held by ASHLYN BOLTON LLC involves inherent risks of injury. These risks may include but are not limited to sprains, fractures, dislocations, or other physical injuries. I affirm that I am physically capable of participating in these activities and assume full responsibility for any injuries or damages that may occur during my participation. I understand that ASHLYN BOLTON LLC and its instructors are not responsible for any medical costs or other expenses related to any injury. Liability Release In consideration of being permitted to participate, I agree to release, indemnify, and hold harmless ASHLYN BOLTON LLC, its owners, instructors, employees, and affiliates from any and all claims, demands, or causes of action arising out of or connected with my participation in these classes.

Photo/Video Release

I acknowledge that ASHLYN BOLTON LLC occasionally captures photos and/or videos during classes and events for promotional purposes. By signing this agreement, I grant ASHLYN BOLTON LLC permission to use my likeness in any promotional materials, including but not limited to social media posts, websites, and advertisements.

Refund Policy

I understand and agree to ASHLYN BOLTON LLC’s refund policy:

– Professional Development Program: We understand that injuries and emergencies can come up that may prohibit you from attending. The $250 Deposit is refundable until April 1st, 2026. No refunds will be given afterwards. Medical issues will be addressed on a case-by-case basis.

– Festival Class Tracks and Bootcamp: If a dancer becomes ill, injured, or emergencies occur, your class will be refunded in 3-5 business days. Please contact tampabaytaps@gmail.com if anything arises. 

– Performance Track: We understand that injuries and emergencies can come up that may prohibit you from attending. The $135 Choreography Fee is NONREFUNDABLE. Festival Class Tracks included in your payment are refundable in the event of illness, injuries, or emergencies. 

– Student Showcase: Withdrawal from the Student Showcase is refundable before June 1st, 2026. 

– COPASETIC: ASHLYN BOLTON LLC is not responsible for ticket purchases made on StrazCenter.org. You must abide by the Straz Center’s Refund/Cancellation Policy. 

I have read and fully understand this Liability Waiver and Release Agreement. By registering for an ASHLYN BOLTON LLC event, I voluntarily agree to all terms outlined herein.
